Admissions


APPLICATION FOR ADMISSIONS

Apply for admission to Cascadia as a new student - online form. If you are not a permanent U.S. resident, do not use this form. Instead, see International Programs.


Underage Admissions Packet

Underage students (16-17 years old) who are not Running Start and seeking enrollment under exceptional circumstances.


 

Records and Registration

 

Assessment Score Request - by email request during remote operations

To request a copy of your Cascadia Assessment Scores, please email enrollment@cascadia.edu with your full name, date of birth, and last four digits of your Student ID number. Requests can take up to 1-3 business days. Only Cascadia scores available.

 

CREDIT REGISTRATION FORM - BY EMAIL REQUEST DURING REMOTE OPERATIONS

Students can register, add, drop, or withdraw online at your ctcLink account before deadlines.


Launch your ctcLink account in the Firefox browser or on your mobile device.


During the late registration period and while in remote operations (while campus is closed and online services are offered), students who receive permission from the instructor can register or add a class by sending a email to enrollment@cascadia.edu. The email must include: 

  1. Full name
  2. Last Four Digits of your Student ID number
  3. Item number of the class
  4. Course name and number
  5. Approval email from the instructor clearly stating their approval with item number, course name, and approval for over-enrollment 

 

Students with holds on their accounts must email enrollment@cascadia.edu with your full name, date of birth, and the last four digits of your Student ID number to address the necessary steps to remove these holds. Requests can take up to 1-3 business days.

The Diploma Reorder Form is for those who lost or misplaced their diploma.

RELEASE OF INFORMATION FORM

Authorizes release of FERPA (Family Educational Rights and Privacy Act) protected student information by Cascadia to a family member, friend, or other person whom you designate. 


Repeat Class Form

After completing a class that you repeat, update your GPA and transcript.


Residency Affidavit

Washington Higher Education Residency Affidavit must meet certain criteria. Submit the Residency Affidavit to the enrollment@cascadia.edu or the Welcome Desk.

ES Petition and hardship withdrawal

These requests-for-withdrawal forms are intended for extreme circumstances only that relate to medical or military-related situations. Please review the instructions on the MS Form in the links below to understand eligibility requirements. These forms cannot be reviewed if turned in after the end of the current term.

To be filled out between the 10th day of the current term to the last day to withdraw.

Tuition Waivers

 

Tuition Waiver for Eligible Veteran and National Guard Member

If you are an eligible veteran or National Guard member, apply for applicable waivers.

If you are the child or spouse of an eligible veteran or National Guard member, apply for applicable waivers.

Adult HS Completion

ADULT HS COMPLETION INFO SHEET

Adults who want to complete requirements to earn a high school diploma. See High School Completion Pathways to help determine your pathway.

STUDENT SUCCESS PLAN

Students carrying five or more credits will be placed on Academic Probation at the end of any quarter in which their quarterly GPA is below 2.0 for a second consecutive quarter and is required to submit the Student Success Plan before registration each quarter to an Academic Advisor (an advising appointments should be made for submitting the form) . This form outlines steps for improving the student's academic performance.

Petition for Readmission

Students carrying five or more credits will be placed on Academic Suspension at the end of any quarter in which their quarterly GPA is below 2.0 for a third consecutive quarter. To be considered for reinstatement, students must submit this form and show proof of circumstances over which they did not have control and/or proof of making measurable and substantial progress towards improving their grade point average. This form must be submitted to an Academic Advisor (an advising appointment should be made for submitting this form).

Student Accessibility Services Accommodation Application

Apply for disability services. This is the first step as a new student applying to the Disability Student Services Office. For details, see Student Accessibility Services.

Student Accessibility Services Quarterly Letter of Accommodation Request Form

Request quarterly for a Letter of Accommodation by completing this request form. Submit this form to Student Accessibility Services in the Kodiak Corner.

Running Start

 

RUNNING START ADMISSIONS PACKET

The Running Start Admissions Packet includes the Running Start Contract is for prospective Running Start Students.

Running Start Enrollment Verification Form

To qualify for registration, complete this form with your high school counselor each quarter and return to the Running Start Office. 

Running Start Fee Waiver Form

For qualifying Running Start students, this fee waiver is for state mandated fees only.  To qualify, a student must be currently eligible for the free/reduced price lunch program, or have been eligible in the past 5 years. A letter from the school district indicating eligibility for the free/reduced lunch program and return with this waiver form to the Running Start Office.

Transportation and Parking

Bus Pass / ORCA Form

To purchase a bus pass / ORCA card.

Purchase Parking Permits online with credit/debit card

Quarterly parking permit, 2-day parking permit, 3-day parking permit, carpool parking permit and motorcycle parking permit are available for purchase online only. To get started, you will need to have a UW NetID. Please note that it takes 48 hours after applying to receive your UW Net ID. The parking permits are now virtual (no window stickers or placards anymore) and you can register multiple cars with in and out privileges. Purchase your parking permit now.

Parking Appeal

Campus Commuter Services enforces all parking and traffic regulations on campus. Failure to pay fines will result in holds on grades, transcripts, refunds or credits, or delay registration for the following quarter. Cascadia forwards unpaid fines to a collection agency.

 

Parking citations beginning with "4491" can be paid or appealed online via ThePermitStore, while citations beginning with "0" can be paid or appealed online via NuPark. If you have any questions, please call Commuter Services at 425-352-3369 or email uwbpark@uw.edu.
