Admissions
Application for Admissions

Apply for admission to Cascadia as a new student - online form. (If you are not a permanent U.S. resident, do not use this form. Instead, see International Programs.)

Underage Admissions Packet Underage students (16-17 years old) who are not Running Start and seeking enrollment under exceptional circumstances.
Records and Registration
COMPASS Score Request Form To request a copy of your Cascadia COMPASS Scores. (Only Cascadia scores available.)
Credit Registration Form To register, drop, or withdraw. For more information about register, drop, or withdrawals, check Academic Policy. For quarterly deadlines, check the Academic Calendar
Enrollment Verification Form To verify your enrollment as a credit student at Cascadia - online form.
Graduation Application To apply for graduation from Cascadia with a degree or certificate.
Name Change Form To officially update your legal name on your student record.
Official Transcript Request To purchase your Cascadia Official Transcript - online form.
Program Update Form To officially update your program of study on your student record.
Release Information Form Authorizes release of FERPA (Family Educational Rights and Privacy Act) protected student information by Cascadia to a family member, friend, or other person whom you designate.
Repeat Class Form After completing a class that you repeat, update your GPA and transcript by submitting this form back to the Kodiak Corner Front Counter.
Residency Affidavit Washington Higher Education Residency Affidavit must meet certain criteria.
Residency Classification Information about eligibility for residency for tuition paying purposes in Washington State.
Residency Questionnaire  To apply for residency in Washington State for tuition paying purposes.
Transcript Evaluation Request To evaluate credits from another college or university and apply the credits towards your Cascadia degree.
Transportation
Bus Pass and Motorcycle Parking Pass Form To purchase a bus pass or motorcycle parking pass. This form may also be used to purchase parking permits via cash or check at the Kodiak Corner Front Counter. Credit and debit card purchases should be made online (see Parking Pass below).
Carpool Parking Pass To purchase a carpool parking pass.
Parking Permit Application

Purchase a 2-day, 3-day, or Quarterly Parking Pass - online purchase. When you purchase the parking permit online, you will be able to print out a temporary permit. The permanent permit will be mailed to you. Avoid standing in line and make it convenient for you. Purchase your parking permit now.

Parking Appeal To appeal a parking citation - online form.
Waiver Forms
Tuition Waiver for Eligible Veteran and National Guard Member

If you are an eligible veteran or National Guard member, apply for applicable waivers.

Tuition Waiver for Child or Spouse of POW/MIA/deceased eligible Veteran or National Guard Member

If you are the child or spouse of an eligible veteran or National Guard member, apply for applicable waivers.

Tuition Waiver for Senior Citizens

If you are a senior citizen, apply for applicable waivers.

Tuition Waiver for State Employees or Public School Teacher

If you are a Washington state employee or a public school teacher, apply for applicable waivers.

For additional waivers, go to the Paying for College page. The additional waivers may not require a form, but will require additional documentation to certify eligibility for the waiver.

Advising and Counseling Forms

 

Adult HS Completion
Adult HS Completion Packet Adults who want to complete requirements to earn a high school diploma.
Academic Advising
Academic Probation Contract Students carrying five or more credits will be placed on Academic Probation at the end of any quarter in which their quarterly GPA is below 2.0 for a second consecutive quarter and is required to submit the Academic Probation Contract before registration each quarter to an Academic Advisor (an advising appointments should be made for submitting the form) . This form outlines steps for improving the student's academic performance.
Petition for Readmission

Students carrying five or more credits will be placed on Academic Suspension at the end of any quarter in which their quarterly GPA is below 2.0 for a third consecutive quarter. To be considered for reinstatement, students must submit this form and show proof of circumstances over which they did not have control and/or proof of making measurable and substantial progress towards improving their grade point average. This form must be submitted to an Academic Advisor (an advising appointment should be made for submitting this form).

Disability Support Services
Disability Support Services Accommodation Application Apply for disability services. This is the first step as a new student applying to the Disability Student Services Office. For details, see Disability Support Services.

Disability Accommodations Request for Proctored Exam Request disability test accommodations such as extra time on exams and proctoring. For details, see Disability Support Services.
Authorization to Release Information on Campus This form authorizes and enables Disability Support Services to expedite and coordinate services for students utilizing email as a method of communication to other areas, staff, or instructors on campus. 
Confidential Information Release Form for Disability Services Authorize release of confidential information to the Cascadia (this is best practice if our office will need to contact an outside agency such as your medical provider or high school for documentation).  For details, see Disability Support Services.
Disability Support Services Guidelines and Requirements for Documentation Students seeking support services from post secondary institutions in the state of Washington on the basis of the disability described in this document may be required to submit documentation to verify eligibility. Opportunity Grant Application To apply for the Opportunity Grant Program. See requirements for the Opportunity Grants.
Running Start
Running Start Contract New Running Start applicants are required to complete the steps and submit this contract by the Running Start application deadlines to the Kodiak Corner Front Counter.
Running Start Enrollment Verification Form To qualify for registration, complete this form with your high school counselor each quarter and return to the Running Start Office. 
Running Start Fee Waiver Form For qualifying Running Start students, this fee waiver is for state mandated fees only.  To qualify, a student must be currently eligible for the free/reduced price lunch program, or have been eligible in the past 5 years. A letter from the school district indicating eligibility for the free/reduced lunch program and return with this waiver form to the Running Start Office.
