How to Apply for Services
There are three simple steps to apply for services. Accommodations will be determined once they have been completed.
- Gather diagnosic documentation of your accessibility need, diagnosis, or condition. Depending on the individual’s accessibility need, documentation can be submitted from a medical doctor, psychologist, psychiatrist, neuropsychologist, school psychologist or other professional diagnostician. Students are encouraged to contact Student Accessibility Services if they are having trouble acquiring documentation. If a disability is apparent and the need for accommodations can be easily established, documentation may not be needed.
- Fill out and submit the Accommodation Application form.
- Schedule a meeting with the Director of Accessibility & Student Support Services.
