How to Apply for Services

There are three simple steps to apply for services. Accommodations will be determined once they have been completed.

  1. Gather diagnosic documentation of your accessibility need, diagnosis, or condition. Depending on the individual’s accessibility need, documentation can be submitted from a medical doctor, psychologist, psychiatrist, neuropsychologist, school psychologist or other professional diagnostician. Students are encouraged to contact Student Accessibility Services if they are having trouble acquiring documentation. If a disability is apparent and the need for accommodations can be easily established, documentation may not be needed.
  2. Fill out and submit the Accommodation Application form.
  3. Schedule a meeting with the Director of Accessibility & Student Support Services.
